You have to submit your details, such as, present name of the candidate, proposed new name, photo ID proof like Passport, Voter ID, Aadhar Card, Driving Licence, Bank Passbook, father's name, mother's name, age as on date along with date of birth, copies of educational certificates, basic as well as highest, birth certificate if available, etc. The name or religion would be changed by Central Government Authorities.
