You have to give your present name, proposed new name, father's name, mother's name, age and date of birth, address as per Photo ID proof such as Aadhar Card or Passport or Voter ID Card or Driving Licence or Bank Passbook etc., along with two latest passport size photographs. In Andhra Pradesh or Telangana States, the procedure is three fold : First they have to approach the Police Station concerned; fill up the prescribed form, get it signed by Gazetted Officer nowadays they are insisting and by paying the necessary fees. Then the police officials make an enquiry and submit their report and based on their enquiry, report will be issued. By enclosing the said police verification report, then the candidate has to approach the Mandal Revenue Officer concerned. There also, the MRO sends the official concerned like Revenue Inspector or so to the address who conducts Panchanama and sends compliance and on that basis the MRO gives his certificate. Then the person has to approach the Gazette Authorities with these two verification reports by paying the necessary fees towards publication charges and get the publication done.
The person is supposed to mention his old name and proposed new name, by way of an undertaking in the prescribed format duly signed by him/her on a non-judicial stamp paper of minimum denomination, followed by a paper statement in a leading local newspaper, and submit his/her two self-attested latest photos, attested copy of I.D. proof containing photo (Passport, Aadhar Card, Voter I.D. etc.), two sets of prescribed proforma with the signature of the person and also two witnesses, Certificate signed by the individual, etc., etc., along with requisite fee by way of Demand Draft etc. For minors, guardians have to apply in the prescribed proforma, giving all the details and documents.
